Client Intake Form
Prior to your first session with us, we ask that you fill out the following form:
The client intake form is for practioner use only. If you are uncomfortable completing any part of the form, please leave that question blank and we can review the necessity for that information together during your first appointment. Documentation, receipts, and other pertinent client information will be released upon client request to assist you if you are filing a health insurance reimbursement claim.
Driving Directions
When you are pulling into the entrance of the condominium complex, please go in slow and at an angle, there is a dip and if your car is low to the ground the front spoiler may scrape.
Traveling from the Easton area:
- Travel down William Penn Highway away from Easton and towards Bethlehem
- Pass Blue, The Christmas Barn, and Nancy Run Fire Co.
- After the Hess gas station and Keystone Pub on the right, make the next right on to Butztown Rd.
- Continue on Butztown as it bends sharp left and continue straight at the 4 way stop. There is a school on the left.
- AFTER the next left, you will see a Brick front house and then a small entrance on the left with two stone pillars that says Stonegate Condominiums.
- Turn left SLOWLY and at an angle into Stonegate Condominiums. I am the last unit on the right, #C6. There is parking across from the unit.
Traveling from the Allentown area:
- Travel 22 East and exit at Rt 191
- Make a left at the end of the exit ramp on Rt 191. Pass Home Depot, Ground Round.
- Continue about 1.5 miles and when you see the 7-11 store on the left make the next left at the light onto Butztown Rd.
- Continue straight at stop sign.
- Very soon after you will see a small entrance on the right with two stone pillars that says Stonegate Condominiums
- Turn right SLOWLY and at an angle into Stonegate Condominiums. I am the last unit on the right, #C6. There is parking across from the unit.
Traveling from the North:
- Route 33 South
- Take William Penn Highway Exit and make a right at the end of the exit ramp.

- Take the Northeast Extension
- Exit on Route 22 – Allentown
- Travel on Route 22 East
- Exit at Route 191
- Make a left at the end of the exit ramp on Rt 191. Pass Home Depot, Ground Round.
